The structure
Full Name
Date of Birth
Address
Home phone number
Cell phone number
"Brief description about yourself."
"Brief description on what skills you have to apply it to the work world."
"Years of experiences(Past jobs or current with address and how long you worked there and whatever you did there.)"
"Colleges attended(ing) or Degrees recieved"
Thats a simple one. Its worked for me.

Three major types
Skills/Personal achievments: New entry, new hire position. Have some related evidence of experience toward the field your entering. college stuff etc
^ with reallife work experience: Other businesses you have worked for etc. Good for job changes in your career or basicly during your career
^^ Both if your some ace stud out of college and can do or have done all the shit they want
Add related important info people above have mentioned and follow a professional template. Pick one there are alot, but make sure you include things in chrono order.
Make your self look fucking good and ace the interview. Be as confident as possible and pull out all the charm. Just get the job, you can be an asshole or whoever you want later lol. The interview is the NUMBER#1 most important thing.

A resume is just a way of convincing someone else that you have a giant cock, but you won't actually whip it out and show them.

These are the things that they can't ask about:
Medical or disabling conditions
Whether or not the interviewee is married, or has plans to marry
Whether or not he or she has or plans to have children
The interviewee's religious beliefs (unless the employer is a religious-based organization)
His or her age
Sexual orientation
Where or when the interviewee was born
Whether or not an employee has ever been arrested (as opposed to convicted)

Thats a simple one. Its worked for me.
I just have my name, address, home number, cell number, and e-mail. I personally don't think you should have your date of birth on it. You should also have a section with accomplishments, and special skills.

ya i'm confused if its illegal why do they got the boxes for them? i'm not sure if this is a federal law and not just some states laws.

I don't know about in the US, but here in the UK you need to add a bit more than basic details. Our structure is along the lines of this...
Name
D.O.B
Address
Home Town
Tel
Shool History (Junior Schools/High Schools etc)
Grades
Work Experience
Previous Employment
Key Skills
Personal Summary
