Cerberus Phreakmods Driver?

I hope I’m posting this in the right forum section,
hiya folks, I seem to be having an issue with an arcade stick I am custom making, which comes as no surprise to me, this is my first attempt at making a custom stick…
I have a Cerberus Phreakmods PCB, and right now I am attempting to check if the USB plug in works. I have the wires placed inside of the USB OUT four slots, and connected the USB end to my PC. My PC can detect it, but it immediately says that it is an unrecognized device, and therefore stopped working. What is it that I am doing wrong? Is there a driver out there that I need to install? If so, where?

I recall seeing a firmware update file in the form of a .hex on the Phreakmods page, but I’m embarrassed to say that I have no idea how to make use of that .hex file.

Any and all help is appreciated, let me know what all I am doing wrong.

You have the green and white wires crossed

I tested it my Self and used the Cerberus board to convert other controllers to USB for PC/Mac use.

Could also not have the USB wires punched all the way too, make sure the wires are all the way back and you can see them in the little windows.
